December 7 is the deadline to select 2016 Medicare plans.

Representatives of Butler’s College of Pharmacy & Health Sciences and Community Health Network will help you choose the right Medicare Part D option to cover your prescription drug costs during FREE clinics this fall.

You can confirm your plan at the clinics, and apply for low income assistance with prescription drug costs.

Call to reserve your one-hour appointment, 9am to 4pm.

Bring the following with you:
•       Medicare and insurance cards
•       List of medications and dosages
•       Letter(s) from Social Security Administration OR Centers for Medicare and Medicaid Services
•       Contacts for your pharmacies or home delivery services
•       New Medicare enrollees who have prescription drug coverage should bring a letter from your union or former employer health plan
